- Please download the WFP Global Producer Set-up packet above. This will get you set up to sell both fixed insurance and fixed annuities with WFP.
- Please note that we need a signature on pages, 5, 9, and 10.
- When completed, please send securely to wfphelp@wentworthfp.com
- Please also include:
- Letter(s) of explanation for any disclosure issues
- Copy of your most recent AML certificate
- A voided check
- Any NAIC state specific certificates you have
- Regulation 187 Acknowledgement Form for all NY Annuity sales
